The Department of Health has issued a VA COVID-19 Vaccine Provider Intent form. Pharmacies who wish to receive COVID vaccine allocations from VDH are required to fill out the form. Questions regarding the COVID-19 Vaccine Provider Intent Form can be directed to COVIDVaccineInfo@vdh.virginia.gov
