On December 30, 2025, the state was awarded $185,974,368 in annual grants to be distributed over five years to implement the plan outlined in its grant application, which focuses on five initiatives: “improve rural access to care through technology; ensure accessible quality care through innovative models; sustain rural workforce with training, recruitment, and retention; implement population-specific, evidence-based projects to make rural America healthy again; and invest in rural health infrastructure and partnerships.” The state will use the funds to provide subgrants to support the goals and projects outlined in its application. For more information about the state’s Rural Health Transformation Program (RHTP) application, visit the Department of Health and Welfare’s RHTP website.

Indiana: The Medicaid proposal to cut pharmacies' reimbursement remains unchanged, but the timeline for stakeholders to comment has been extended until October 27. A public hearing is scheduled on the same date to allow for public comments. 

 

Also in Indiana, to address a $250 million Medicaid budget shortfall, the Family and Social Services Administration (FSSA), Office of Medicaid Policy and Planning (OMPP) is proposing to reduce the pharmacy dispensing fee from $10.48 to $9.63. This proposal is based on the 2024 survey conducted by Myers and Stauffer. If approved by the Centers for Medicare & Medicaid Services (CMS), the new reimbursement rate would take effect on March 1, 2026. NACDS is submitted a comment letter objecting to these proposed cuts.  

Finally in Indiana, the Family and Social Services Administration (FFSA) Health Coverage Programs (IHCP) provided updates approved by Drug Utilization Review Board at the August 2025 meeting.

The Department of Insurance (DOI) published its June 2025 newsletter. As a reminder pharmacy providers may submit inquiries and complaints on the DOI website. Complete a separate form for each PBM if there are multiple complaints. To date, the DOI has received 43 complaints from pharmacies regarding PBMs. Of these, 36 remain open or pending, while seven have been closed. Complaints may include multiple issues, with the most common being related to Maximum Allowable Cost (MAC) appeals and dispensing fees. The DOI Consumer Affairs team is also assisting the PBM Examiner with investigating reported concerns. For questions and information, please contact PBM@doi.idaho.gov.

Myers and Stauffer is the vendor contracted to conduct the annual prescription volume survey of pharmacy providers on behalf of the Department of Health & Welfare. In accordance with the Idaho Administrative Code 16.03.09.665.01j, the prescription volume data collected from pharmacies will be used to determine their dispensing fee tier for the following calendar year. Each year, surveys are distributed starting May 1 and will be due by May 31. Providers who fail to participate in the survey will be placed in the lowest dispensing fee tier of $11.51. Below are the current dispensing fee tiers based on Total Annual Prescription Volume: 

  • Fewer than 40,000 claims: $15.11 dispensing fee 
  • Between 40,000 and 69,999 claims: $12.35 dispensing fee 
  • 70,000 or greater claims: $11.51 dispensing fee 

Please note that the requested information applies to total pharmacy volume and is not limited to prescription/sales information related to Department of Health & Welfare recipients unless otherwise indicated. 

2025 Idaho Prescription Volume Survey 

Providers should fill out the survey form for all pharmacy NPIs within their organization that are enrolled with Idaho Medicaid. For providers with 5 or more pharmacy NPIs, please upload via the “multiple store” submission option at the survey link above. Also in Idaho, Myers and Stauffer is working with the Department of Insurance to conduct a survey of costs incurred by pharmacies to dispense prescription medications to Idaho residents. All pharmacies that dispense prescriptions to Idaho residents are requested to complete the pharmacy dispensing fee survey.  

Myers and Stauffer mailed a packet with survey information to pharmacies on April 17. Providers can also access the survey documents and download an electronic version of the survey tool located on the Dispensing Fee Survey webpage 

It is very important that all pharmacies cooperate fully by filing an accurate cost survey.  Pharmacies are encouraged to return the requested information as soon as possible, but forms must be returned no later than May 29.

Alex Adams, former executive director of the Board of Pharmacy, has been tapped to serve as the US Department of Health and Human Services assistant secretary for family support. The US Senate, which confirms presidential nominations, received his nomination. The State Legislature introduced a House Joint Memorial No. 8 to honor his work in the state.

Gov. Brad Little (R) signed HB 231, legislation expanding grocery tax credit to offset the sales tax Idahoans pay on food. The new law increases the grocery tax credit to $155 a year, an increase above the current amount of $120 per year.

NACDS submitted comments to the Senate Health Care and Wellness Committee in support of SB 503, which includes to ensure network adequacy, requirements that PBMs reimburse pharmacies at a minimum of the acquisition cost of a drug and an adequate professional dispensing fee (PDF) equal to the Medicaid fee-for-service (FFS) PDF. Also, the bill prohibits steering patients, defines specialty drugs, prohibits certain fees, and provides additional oversight and enforcement in the Office of the Attorney General.

Also in Indiana, SB 140, a more limited PBM reform bill modeled after the Kentucky law, passed the Senate Health Care and Wellness Committee and was referred to the Senate Appropriations Committee for further action. This bill requires the Insurance Commissioner to determine minimum reimbursement for a professional dispensing fee based on a cost of dispensing study done every two years. It also requires PBMS to file an annual report on adequate networks and establishes a process for filing complaints.

Also in Indiana, HB 1606, a bill prohibiting spread pricing, was referred to the House Insurance Committee.

Also in Indiana, SB 96, immunization clean-up legislation addressing age restrictions and the list of immunizations allowed to be administered by a pharmacist was referred to the Senate Public Policy Committee.

Finally in Indiana, HB 1571, a scope of practice expansion and clean-up bill, address multiple issues including immunizations, Test and Treat, pharmacy technician duties, MAT and payment for services. The bill was referred to the House Public Health Committee.
